A "cat back" system is an exhaust kit that replaces everything from the factory catalytic converter with new piping from the "cat" out to the tail pipe. Your catalytic converter stays in place.
For much less money, and the same results, you can replace the factory muffler with a higher performance one. You can also replace the "cat" with a high flow performance "cat".
With a cat back system you can also increase the size of the piping from the cat out to the tailpipe.
